To apply as a vendor for USAG Humphreys Family & MWR events, the following documents are necessary:
1. **USAG Humphreys Family & MWR Vendor Application Form**:
- This form must be completed and emailed to **[email protected]** by the published deadline.
- Incomplete applications will be returned.
- The form requires details such as:
- Organization name
- Point of contact (POC) name, phone number, and email
- Event name
- Electricity request
- Vendor purpose
- List of food/items intended for sale
- Vendors must also agree to the terms, conditions, and statement of liability outlined in the application.
2. **Approved DD Form 2970 (Application for Temporary Food Establishment)**:

This form is required for entities selling food and other consumables.
- It must be submitted to the regulatory authority at least **7 days before the event**.
- The form includes details such as:
- Name of the temporary food establishment
- Name of the operator or owner
- Mailing address, telephone number, and event details
- Food and beverage items to be prepared and served
- Food preparation methods, storage, and transportation details
- Information about handwashing facilities, potable water supply, utensil washing, wastewater disposal, and garbage disposal
- Additional attachments may be required:
- **Attachment A**: Food preparation details at the temporary food establishment.
- **Attachment B**: Food preparation details at a permanent food establishment supporting the temporary food establishment.
- **Attachment C**: Temporary food establishment employee log.
3. **USAG Humphreys Fundraising Request Form** (if applicable):
- Required for Soldier and Family Readiness Groups (SFRG) and Private Organizations (POs) seeking command approval for fundraisers or receiving donations.

- Approved applicants must make payment within **5 working days** of notice to secure their vendor space.
- Vendors are responsible for providing their own materials, equipment, and setup for their booths.
- Vendors must comply with all terms and conditions outlined in the application form, including restrictions on alcohol, to***co, and subletting of vendor space.
